The DW19W is a 2-window envelope for laser tax forms. Use with 3-Up pre-printed, computer drawn, or continuous 1099, 1098-E, 1098-F, 1098-T, 3921, 3922, 5498-ESA, 5498-SA forms. Not all envelopes are sized for all forms, and using the incorrect ones can compromise sensitive employee data as well as delay or prevent delivery. Please be sure you are ordering envelopes that fit the forms you are using.
